How it works

Apixaban is a direct, selective and reversible inhibitor of Factor Xa. It works by blocking Factor Xa activity, which is an essential enzyme in the blood clotting process.

By inhibiting Factor Xa, Apixaban reduces thrombin generation, prevents fibrin clot formation, and lowers the risk of harmful blood clots in veins and arteries.

It does not require antithrombin III for its action and has no direct effect on platelet function, but indirectly reduces platelet activation by decreasing thrombin production.

Usage and Safety

Dosage

• Atrial Fibrillation (Stroke prevention): 5mg twice daily
• Dose may be reduced to 2.5mg twice daily in elderly or low-weight patients or those with renal impairment
• DVT/PE Treatment: 10mg twice daily for 7 days, then 5mg twice daily
• Prevention after surgery: 2.5mg twice daily

Side Effects

• Bleeding (nose, gums, urine, stool)
• Bruising
• Anemia
• Gastrointestinal bleeding
• Eye bleeding
• Rare: Brain hemorrhage
• Low blood pressure due to blood loss

Seek medical help immediately in case of severe bleeding.

Drug Interactions

• Strong CYP3A4 and P-gp inhibitors (ketoconazole, ritonavir, clarithromycin)
• Inducers (rifampicin, carbamazepine, phenytoin, St. John’s Wort)
• Aspirin and other antiplatelets
• NSAIDs (ibuprofen, diclofenac)
• Heparin and other anticoagulants

Apixaban is used for prevention and treatment of blood clots including:

• Stroke prevention in atrial fibrillation
• Deep vein thrombosis (DVT)
• Pulmonary embolism (PE)
• Post-surgical clot prevention

When Not to Use

• Active bleeding
• Severe liver disease with bleeding risk
• Hypersensitivity to apixaban
• Recent brain, spinal or major bleeding events
• Not recommended in prosthetic heart valve patients

Precautions

Use with caution in:

• Kidney impairment
• Elderly patients
• Patients at high risk of bleeding
• Before surgical or dental procedures

Warnings

• Risk of serious or fatal bleeding
• Do not stop suddenly without doctor advice
• Spinal/epidural hematoma risk during spinal procedures
• Requires careful monitoring in high-risk patients
