How it works

There is evidence that malfunctioning of glutamatergic neurotransmission, in particular at N-methyl-D-aspartate (NMDA) receptors, contributes to both expression of symptoms and disease progression in neurodegenerative dementia. Memantine is a voltage-dependent, moderate-affinity uncompetitive NMDA-receptor antagonist. It blocks the effects of pathologically elevated tonic levels of glutamate that may lead to neuronal dysfunction.

Usage And Safety

Dosage

Adults: The maximum daily dose is 20 mg per day. To reduce side effects, the dose is increased gradually over 3 weeks:

• Week 1: 5 mg daily (morning)
• Week 2: 10 mg daily
• Week 3: 15 mg daily
• Week 4 onwards: 20 mg daily (maintenance dose)

Elderly: 20 mg per day.
Always use as directed by your physician.

Side Effects

Common: Headache, sleepiness, constipation, dizziness, balance disorders, shortness of breath, high blood pressure, drug hypersensitivity.
Uncommon: Fatigue, fungal infections, confusion, hallucinations, vomiting, abnormal gait, heart failure, thrombosis/thromboembolism.

Drug Interactions

L-dopa, dopaminergic agonists, anticholinergics, antispasmodics, dantrolene, baclofen, amantadine, cimetidine, ranitidine, procainamide, quinidine, quinine, nicotine, hydrochlorothiazide, dextromethorphan, oral anticoagulants.

Indication

Treatment of moderately severe to severe Alzheimer’s disease.

When not to Use

Hypersensitivity to memantine hydrochloride or any ingredient. Not recommended for children and adolescents under 18 years.

Precautions

Caution in patients with epilepsy or seizure history.

Warnings

1. Avoid use with other NMDA antagonists (amantadine, ketamine, dextromethorphan).
2. Patients with recent myocardial infarction, severe heart failure, or uncontrolled hypertension should be closely supervised.
3. Alzheimer’s may impair driving ability; caution is required when operating machinery.
