How it works

Apixaban is a direct Factor Xa inhibitor. It blocks Factor Xa activity, which is essential for blood clot formation. By inhibiting this pathway, it reduces thrombin generation and prevents formation of harmful blood clots.

Usage And Safety

Dosage

Stroke prevention in atrial fibrillation: 5mg twice daily (standard dose)
Dose may be reduced to 2.5mg twice daily in selected patients (elderly, low body weight, or kidney impairment)
DVT/PE treatment: 10mg twice daily for 7 days, then 5mg twice daily
Always use as directed by physician

Side Effects

Common: Bleeding, bruising, nosebleeds, gum bleeding, urinary bleeding, gastrointestinal bleeding
Uncommon/Rare: Eye bleeding, coughing blood, internal bleeding, brain hemorrhage (rare but serious)

Drug Interactions

May interact with anticoagulants, antiplatelets, NSAIDs, thrombolytics, and CYP3A4/P-gp inhibitors or inducers (e.g., rifampicin, carbamazepine, phenytoin, ketoconazole, ritonavir).

Indication

Used for prevention of stroke in atrial fibrillation and treatment/prevention of deep vein thrombosis (DVT) and pulmonary embolism (PE).

When not to Use

Not suitable in active bleeding, severe liver disease with bleeding risk, hypersensitivity, or conditions with high bleeding risk (e.g., recent brain bleed or major surgery).

Precautions

Use with caution in patients with kidney/liver disease or those at risk of bleeding. Not recommended in patients with prosthetic heart valves.

Warnings

Stopping suddenly may increase risk of blood clots. Risk of serious bleeding including spinal/epidural hematoma during spinal procedures. Emergency medical attention required if bleeding signs occur.
